Health Smarts offers information and tools designed to help people with diagnosed conditions stay as healthy as possible for as long as possible.

Online health and wellness assessment

This confidential online questionnaire will help you see your potential health risks and provide a plan on how to lessen or eliminate them. If you get your numbers beforehand, your results will be more meaningful.

ConditionCare/Health Management Programs

Health Smarts offers free, confidential, ongoing condition management services to anyone living with asthma, diabetes, heart failure (HF), coronary artery disease (CAD), or chronic obstructive pulmonary disease (COPD). A nurse who is knowledgeable about your condition will phone you at a time convenient for you to answer your questions, and work with any issues you may be having. These programs supplement the care you’re receiving from your doctor.
